If you are going to register for one of the sessions of the STRICTLY HOCKEY Hockey School, please FULLY COMPLETE and SIGN the form.

IMPORTANT POLICY INFORMATION REGARDING REFUNDS AND WAIVERS!

Refunds
Please Note: Refunds will be made up until 2 weeks before the beginning of a camp/ clinic, subject to a $25.00 administration charge. Cancellations made after that point will be credited to enrollment in the school for the 2014 sessions or subsequent clinics.

Waiver
Waiver– I give my approval to my child’s participation in all activities of the STRICTLY HOCKEY Hockey School and agree that STRICTLY HOCKEY, its owners and employees will not be responsible for any damage or accident (medical or dental) incurred while participating. I do waive, release, absolve and agree to hold harmless STRICTLY HOCKEY, its owners, employees and volunteers in such instances. I hereby acknowledge that I have read the terms and conditions and certify that the applicant(s) is in good physical health.
